Skilled Nurse Coordinator
Purpose of the Position
A unique and versatile position in the Skilled Nursing/Terrace View Neighborhood of the Friendship Haven campus.
Essential Duties
· Responsible for coordinating the care needs for the skilled nursing residents from admission to discharge.
· Complete admission/discharge paperwork for each resident, making necessary discharge referrals, identifying and implementing education/teaching in preparation for discharge and other miscellaneous duties relating to discharge needs.
· Accountable for completion of identified skilled MDS interviews/sections, CAAs and development of associated care plans.
· Set up and attend skilled care plan conferences for the skilled residents.
· Provides education and performs care reviews to neighborhood team members.
· Assists the Coordinator with residents/family/staff concerns, as needed.
· Assists neighborhood in day-to-day nursing staff vacancies, as needed and able.
· Assists in other various opportunities, as able and necessary, to meet the care needs of the skilled care residents.
· Is required to be on call for in the nursing administration rotation schedule.
· Provide Social Services such as one-on-one counseling, depression screening, reassurance and mental health referrals, as needed.
· Explain and provide required Medicare notices per federal and state regulations to skilled residents.
· Work with various outside Agencies, as needed.
· Provide financial counseling as needed by residents or family.
· Develop and update educational materials that can be utilized for teaching and provided to the Skilled Neighbor upon discharge.
· Serves on Friendship Haven Committees, as needed.

Minimum Requirements
· Education: Licensed RN, a background in Human Services is beneficial but not required.
· Must be CPR certified.
· Long Term Care experience preferred, but not required.
· MDS certified beneficial, but not required.
Physical Demands
· Sits, stands, bends, lifts and moves intermittently during work hours.
· Willing to work beyond normal working hours, and on weekends and holidays when necessary.
Mental Demands
· Subject to frequent interruptions.
· May be subject to hostile and emotionally upset residents, families and visitors.
