Purpose of the Position

Provide a broad range of services to meet the individual needs of each resident’s social, environmental, and personal care in the residents’ home enhancing the capacity of residents to attain or maintain their highest level of independence, offering choice and control. Provides general guidance and oversight to Caregivers and Versatile Workers as part of the Assisted Living leadership team.

Essential Functions

  • Provides leadership to the program by modeling professionalism, customer service, collaboration, flexibility, and acts as a coach for resident-centered care.
  • Works effectively as part of the neighborhood team accepting responsibility for work-related tasks and working with other departments to coordinate resident care.
  • Assists the Program Nurse and Leadership team in planning programming that meets residents’ needs and adapts approach to anticipate future needs, for both the general assisted living population and the dementia population.
  • May provide personal care assistance including grooming and other assistance as needed. Knows the content and follows each resident’s service plan.
  • Develops effective relationships with residents and families, learning resident preferences, and offers input in individualizing services for residents based upon their bio, history, and activity preferences.
  • Responds appropriately and timely to resident requests for help and maintains positive relationships with resident’s friends and family members. Creates and maintains an atmosphere of warmth and personal interest, as well as a calm environment.
  • Provides residents with opportunities for independence and choice consistent with their Service Plan, involves residents in self-care activities. Promotes independence and wellness.
  • Communicates effectively and appropriately with physicians, medical professionals, and family members regarding residents’ condition and under the guidance and direction of the Program Nurse(s).
  • Responsible for assisting with schedules for all assigned staff, assisting with coverage requests, providing backup when needed, and bi-weekly timecard processing, as delegated.
  • Assures resident rights are respected and protected. Maintains residents’ self-esteem, privacy, and confidentiality of personal information. Minimize resident losses while maximizing their abilities and strengths.
  • Ensure the cleanliness and attractive appearance of the neighborhood working in conjunction with the household staff.
  • Assists Caregivers with resident cares and rounds as needed.
  • Coordinates and/or completes resident biographies/history and updates as indicated.
  • Follows general safety, first aid, emergency response, privacy policies, and infection control procedures.
  • Communicates and documents services and care in an effective, timely manner in accordance with the Service Plan and policies and procedures. Passes information on to co-workers to provide continuity of service through program huddles, reports, documentation and mentoring.
  • Responsible for ensuring Caregivers and Versatile Workers are following policies and providing resident cares per the noted Service Plan.
  • Responsible for having daily contact with all staff in their training period to review packet and discuss questions or concerns, including monitoring of the person providing the training. Mentoring and coaching for all employees.
  • Will assist program nurses in planning and presenting team meetings.
  • Scheduled hours may vary based on program needs.
  • Participates in on-call rotation for the Assisted Living Neighborhoods.

Knowledge, Skills, and Abilities

  • Knowledge of a resident-centered model of service delivery within the neighborhood.

  • Ability to work effectively with co-workers in a team environment.
  • Proficiency in writing and communicating with other team members.
  • Accountability to complete tasks with minimal supervision.
  • Ability to deliver excellent customer service and public relations. Ability to be tactful in resolving customer service concerns and situations.

Minimum Requirements

  • Licensure: CMA required.
  • CPR Certification, or the ability to become certified.
  • Must be willing to complete cross-training to complete versatile assigned work tasks.
  • Must complete interactive and ongoing dementia training.
  • Must complete Medication Manager course with nurse delegation to administer medications.
  • Required to complete safe food handling and sanitation.
  • Experience: Working in aging-related environments, public service, housekeeping, or dining environments preferred.

Physical Demands

  • Sits, stands, bends, lifts, and moves during work hours.
  • Should be capable of lifting up to 50 pounds on a regular basis.
  • Willing to work beyond normal working hours, and on weekends and holidays when necessary.

Mental Demands

  • Subject to frequent interruptions.
  • May be subject to hostile and emotionally upset residents, families, and visitors.

This is a full-time position. Shift will be 3 pm - 11 pm with an on-call rotation.
